OK.
Lets name the pins from top to bottom and from left to right as follows:

1 2 3 4
A B C D
5 6 7 8

When the lever is down (bridge position) contacts are A1, B2, C3 and D4
When the lever is in the middle (both) contacts are A1, B6, C7 and D4
When the lever is up (neck position) contacts are A5, B6, C7 and D8

Ok. Simple.

Solder the neck positive to B
Solder the bridge positive to A
Solder a jumper between 1 and 6.

Lugs 1 and 6 are your signal output. Wire whichever one to the volume / tone / jack network.

Hermetico, thanks man. Almost there. Neck and bridge are working, but middle position is silent. Any suggestions?

Be sure to use an on/on/on switch.

Also, check whether the switch is Japanese rather than American. (Some manufacturers arranged the internal contact mechanism differently to others.)

Using Hermetico's contact labelling, my version of this wiring would be, neck PU to 1 and 2, output to A and B, bridge PU hot to 5 and 6. The remaining two poles are available for duties such as automatic coil splitting.

If I recall it, this is an Ibanez 4PDT and switches as described above. Not the common 4PDT.

hermetico described it correctly if the URL is for the actual switch. If the pairing was off (AB/CD instead of AD/BC, you'd miss one of the ends, not the middle). I'd check the switch with a multimeter to make sure it isn't on/off/on.

The switch is On/On/On. I put neck and bridge and the jumper as recommended, and the output on 1. Everything is working nicely, but the Middle position is dead. Im kind of baffled. >:P

Muffled or thin or anything else might have other options, but the only way for nothing to come out is a disconnection. The only other suggestion I can come up with is to check that the switch is functioning properly by checking the connectivity of the poles in the different switch positions with a multimeter.

If its important, this isn't an Ibby switch, I actually got this one from Allparts. So, maybe its different than the spec's that Hermanetico suggested?
I checked the continuity between all three, and it is indeed On/On/On...and the two poles I have jumped for the center, they ring out. But when I have it engaged, its dead silent. Could it be the pickups possibly? They are Lollars with 4 wire...I have them wired as Lollar spec'd with the screw ends "out".

Ibanez has its own way, as shown in the last picture.

So, yes, is damn important to know HOW your particular switches in the middle position to provide you the right design.

Do you have some multimeter, to check which lugs are active when in the middle position?.
If not, just try what Funkfingers suggested, since the most common 4PDT on/on/on is somewhat Type 2 and, Funkfingers design will work there.

Awesome...glad I asked. Funkfingers design was correct. Now I have both pickups engaged in the center...which lugs would need to be jumped to tap the center coils instead of both?
