5-way Switch Setup

I'm looking to change out my entire wiring and putting in a 5-way switch and 1 volume. I need to know the type of switch I will need and wiring diagram to have the following positions for HH.

1) Bridge
2) (Bridge & Neck) Outer Coils
3) (Bridge & Neck)
4) (Bridge & Neck) Inner Coils
5) Neck

Or a pair of Duncan TripleShot mounting rings and a regular 5-way.
