I'd just use a switch to combine the bridge pickup into the mix when the 5-way is in the neck or neck+middle positions if I wanted to get all 7 pickup combinations. The switch can be a separate toggle or a push-pull pot type.
This is what Fender did on a late 90's American Deluxe Tele and this link is to the wiring diagram.

What about a 3 position switch for the neck and bridge, plus an on/off switch for the middle?

This is so simple it's stupid....LOL You just need to add in one on/off SPST toggle switch. Position 4 will give neck/middle/bridge and in the bridge (position 5), you'll get neck/bridge.

Nope, one blade switch and 2 3-ways. Also accounts for some splitting IIRC.

Keep it simple. Just get a 5 position Schaller Mega Switch:
1. neck
2. neck and middle
3. neck and bridge
4. middle and bridge
5. bridge
The middle pickup alone isn't that great of a tone and all three on at the same times isn't that great either.

Of course that is only your personal preference. I love both the ones you don't think are so great.

When I was a kid I used to fantasize about all these possible combinations and switches and blah, blah, blah...simpler is better.

You could always go for the channel option.
3 mini 2-way switches plus 1 regular two way. The idea is that the normal one selects between parallel and series. The smaller ones assign each pup to the channel
o. neck switch
o. middle switch
.o bridge switch
O. channel switch
In this example only the neck and middle would be on and in parallel (for instance), while the bridge would be off and in series. If the main swicth was moved you would just have the bridge and in series.
Great for people that want versatility but don't care about looks or fumbling. Then again if you have a select few favourite tones you can just move it inteligently.
I think guitar nuts is the genesis for this.

How many poles and positions would i need for each pickup?

Are you talking about using single coils or humbuckers? A single can only be in/out of phase, series, or parallel with another pickup, and you can't split singles.

That's called "tapping", not "splitting". You can tap at half-way or anywhere else in the wind.

That sounds like a lot of trouble for nothing. Do a test bed and see if you really like those positions.
I've told this before but I used to want all kinds of stuff in a guitar. 3 motherbuckers, splitting all coils, blending them in, volume and tone for all, series, parallel, phase... somethings are just not possible, practical or good sounding. I didn't even get to try such a thing, the price and the realisation that such a thing would be hell to control didn't seem to be worth it.
Sometimes less is more. Go for Artie's suggestion if you just want more combo's, maybe add a few push pull pots to get you series parallel if you want to mess with that.
