If I have a DC-2w and put a guitar signal in input 1 and an octave down of that same signal in input 2, what happens if I just use a single mono output?

I'd assume when the pedal is on they would both be effected and then output through output one, and when bypassed the signals would sum into output 1.

Is this true?

i dont know if thats true... id think you if you use both inputs, you need to use both outputs. if you have one, try it and let us know! you wont hurt anything

You can't use stereo in / mono out. It's not designed that way (per the manual). You can do mono in / stereo out or stereo in / stereo out.
