HH Pearly Gates with 5-way switch.

Can anyone with a diagram writer help me out? I am putting a pair of nickel cover Pearly Gates humbuckers in my Stratocaster. No middle pickup just HH. I'd like the wiring to be as follows

Switch

1. Neck
2. Neck+Exposed Bridge Coil
3. Neck+Bridge
4. Bridge+Exposed Neck Coil
5. Bridge

Pots

Master Volume
Neck Tone
Bridge Tone

You'll need a Superswitch. I'll try to whip something up, but I'm dealing with trying to get all of my old "guitar symbology" into my new paint program.
