Out of phase? Lil 59 pickup not sounding perfect on 2016 American elite stratocaster.

I put the lil 59 neck pickup on the bridge position on my american ELITE strat i adjusted the height to get a desireable output . I like the tone its nice and fat . I have an american elite stratocastwr with n4 noiseless pickups . The 4th position i.e middle and bridge always gave me good tones when i had the bridge single coil but now i the 4th position it is sounding flat. I think its out of phase. It sounds good with the s1 switch on and it sounds good with just the bridge and all the positions sound good. But when the 4th position is on alone without s1 switch it sounds bad. Its connected using the seymour duncan standard wiring. Pls send me a proper color chat for the wiring for the pickup on my start so that i get good tones again on the 4th position.
The 4th position is very trebley and sound flat and bad.
Suggestions..needed pls.. a pickup wiring diagram may help too! Pls help needed

If a humbucker is out of phase with a pickup next to it, you swap black and green.

Out of phase will be half volume not just different sound that could be caused by the new pup.

I am someone new to the "out of phase" aspect, does it happen often for it to go out of phase after being used for a while, or is it only possible to be out of phase initially when you first wire them in and it's wired incorrectly?

No, it won't go out of phase on its own without you changing the wires or magnets.

Swap black and green on the Little 59 (bare still to ground). Fender pickups and Seymour Duncan pickups are out of phase with each other. This is because Fender changed their polarity of their pickups over the years. Duncans are made the way Fenders used to be made. I wish there were some standards for this, but that is unlikely to ever happen.
