You definitely require a DOUBLE POLE (six contact) switch.

Actually you only need two contacts because the only thing you are doing is connecting the hot and ground from the output jack. So when they are separated the guitar functions normally and when the hot and ground are connected the guitar is muted. That diagram is the most complicated way to wire a killswitch. All you need to do is connect a wire from the hot and ground of the output jack and connect one to either of the center lug and the other the either top or bottom lug of the same side, that's it. So only two lugs need to be soldered.

If you have only 1 hum and 1 vol pot then wire it ecaxtly like that and then take two wires, one from the ground lug and one from the hot lug of the output jack and solder one to the center lug of your three pole switch and the other to either outer lug.
