setup problems...help

I recently decided to work on the setup on my les paul copy and have a few questions. When I took the strings off to restring it, the tune-o-matic bridge fell off and now I don't know if the adjustment screws should face the neck or the body. I put it back with them facing the tailpiece, is that wrong? Next, I am having trouble with my high E at the 14th fret. When I set my action to 3/64" it sounds like I am fretting at the 15th instead of the 14th. If I raise my action a little it goes back to sounding pretty close to right. Could my 14th fret be worn too low? Or should I do something with the truss rod, bridge, etc...? The last thing I am having trouble with is my G. If I fret a D chord it sounds out, but an Am sounds good, so WTF. Any help would be appreciated. Thanks in advance.

I think traditional gibsons lP have the tune o matic adjusmtent bolts facing hte neck for some stupid reason - that might accoutn for your intonation problems
othewise - i think john hit it on th enose
