Super switch wiring question

My strat has a hum/single/single setup with a master volume and a push/pull master tone that splits the bridge humbucker. I don't know if it is possible and I can't find a wiring diagram for this setup, but here is what I would like out of the super-switch on my strat. Position 1 would be the switch all the way up.

1 - neck and middle in series
2 - neck
3 - neck and middle in parallel
4 - bridge and middle in parallel
5 - bridge

If anyone can direct me to a diagram or walk me through how to wire this up I would really appreciate it.

Shouldn't be too difficult. Late me take a shot at it this weekend. Then again, there are others that might beat me to the punch. (Hermy?)

Thanks so much. I'm sure my tech could figure it out once he gets under the hood but time is somewhat of a factor and I'm trying to get any info I can to help out.
