Where can I buy a baseplate for Dog-ear P90 pickup?

I have a vintage Dog Ear (P90) pickup. Unfortunately, it's a BRIDGE position dog-ear, so its base-plate is too shallow, thus, the pickup does not sink deep enough in the NECK position of my guitar (making the guitar strings touching the screw pole pieces).

Where can I buy a base-plate that is deep enough so that this dog-ear will sink deeper into the neck pickup route?

Also, where can I buy a cover?
