I have been looking all over: does anyone have a wiring diagram or link for a '65 Melody Maker, SG shape, with 2 single coils, 2 volume, 2 tone and a 4 way slide switch? Model is MM-D. I'm restoring one for a great local player & I was given a bag of parts, literally.
I believe it's the same as the Firebird III, though double check the pot / cap values of the actual guitar before replacing. (I seem to recall the pot values were different?)

Regarding the slide switch. The fact that this has eight terminals does not necessarily indicate that it has four working positions.
The Fender Mustang employs two eight-contact, on-on-on switches. On these, the centre "off" position is acheived by the jumper connections between the terminals. (The sliding contacts bridge the terminals in groups of four.)
If your switch really is four-way, the obvious thing to build would be,
Bridge PU only
B + N in parallel
Neck PU only
B + N in series
Everything else in the circuit can follow Gibson conventions.
