And yes, it will. Your SH-5 would need to be a 4 conductor version (they come stock that way), and I am not sure if SDs pickups are in phase with PRS. If they are not, you'd have to switch the hot and ground. Red & white of the SD would go to the switch on the pot (if that switch is a coil-cut switch).

Yes, but you would have to find the equivalence of what the wire color codes do for each pickup.
