Between the No More Tears name and sound in the video, it makes me think this is or was supposed to be a Zack Wylde signature wah. Thoughts?

Sounds similar in concept to the Fulltone WahFull I picked up this summer...
...3 position rotary switch for VOX Clyde, Crybaby, and one other which I don't recall the name.
The Fulltone WahFull combined with a MOOG foot controller gives me everything and anything I'd need wah-wise.
It can be used as a cocked-wah with very precise settings via the big pot(entiometer), plus the 3 different wah-wah thing with the foot controller,
but with a bigger sweep range if needed.

Mike’s settings are “Whacked” (lots of low end) “Jimi” (classic late 60’s) and “Shaft” (brighter colorsound).

No idea. I have never heard Dave even mention doing anything for Zakk, but who knows. As far as I know, Zakk has a pretty good deal with Dunlop.
I still have a wah that Mike (XSSIVE) built for me, and it does what I want.
