Welcome to the forum, you can get a basic Jazz Bass wiring and with a few mods and have what you want.
First, you need to attach the hot wire of the middle pickup to the same volume pot (the 250k one) as the diagram says for the neck pickup.
Then, instead of wiring the bridge pickup like the diagram says, you can wire it to the push-pull as such:
The + goes to the bridge volume and the - goes to ground.
And then all you have to move the pots around, which us easier to do as the first step, but if you have long wires you can do it afterwards too, but it won't look as nice. You might also have some phase issues depending on what singles you use, if so just switch the + and - wires on the push pull.
