Do you have a multimeter? If you do can you take a measurement for us? Otherwise its just a guess... its a neck pickup from the logo orientation and short legged but that doesnt narrow it down much a resistance measurement would be VERY helpful

To expand upon Edgecrusher's post, measure with the red and white wires connected together. Measuring the ohms from the black and green wire. Your measurement should be from 7-18k (thousand) ohms. It will likely be toward the lower end of the spectrum (7-9K ohms) being a neck pickup.

Measure resistance (ohms) between the red and green wires, and also between the black and white wires.

Sorry without a multimeter there is no way to positively identify this pup.... at this point there are at least 6 different models it could be.

If you get a better picture of the tiny bobbin holes on top, you should be able to see if the wire is purplish (plain enamel) or orangish (poly wire). That should narrow it down some more... Also you can unwrap the tape a little to see if it uses a wooden or black plastic spacer. But the dc resistance is going to be the final indicator.

Actually I take that back, if it was a model that used plain enamel or wooden spacer it probably wouldn't have that 12 hole baseplate.

If I had to guess by nothing more than the photos, it's a Pearly Gates Neck.
