Ibanez 3SWLSC 5 way switch

Can anyone show or tell me how to wire a set of SH-4’s to an Ibanez 3SWLSC 5 way switch? I’ve been pulling my hair out trying to figure it out.

Guitar is Ibanez RG6003FM

With stock wiring

Neck side (as written on the PC board)
GND = Bare
"UPPER" COLD = Green
"UPPER" HOT = Red
"LOWER" COLD = White
"LOWER" HOT = Black

Bridge side
OUT = Volume Pot Input
GND = Volume Pot Ground
GND = Green+Bare
TAP = Red+White
HOT = Black

I wired it in that way, still getting terrible buzz when the strings are muted.

Any chance the jack is wired backwards?

You get buzz in all switch positions? Did you remember to put the bridge (not the pickup, the string bridge) ground back to the volume pot?

The only connections I unsoldered were from the stock pickups to the switch, and the stock neck pickup to the volume pot..

Did you remember that the pickup bare wires ALWAYS go directly to ground on a pot casing, never via a switch?

I found this on the google search. I have the same 5 way switch as described above and two sd black winter neck and bridge
this is what i found
is this correct diagram or its wrong? Shoud i try the diagram posted abive or this one?
