Need wiring diagram for SG with phase mini toggle and 2 push pull pots

The guitar is actually wired up, just need to reinstall a bridge pickup. Thought it was shot, then I did some work to the bridge and neck and brought it back to life.

Has typical SG 3 way toggle, two humbuckers. Two normal volume pots, two push/pull tone pots that split the coils, and I have a mini toggle for phase. I remember when I did this, someone drew it out for me on this site and it worked perfect, but I can't find the post, and I can't find a diagram for this anywhere on the internet. Neck pickup is installed and working fine; I may not even need a diagram if someone can just type out where the four wires on the bridge pickup would go, that would be great. Help! It is like a half Jimmy Page. I would just look at a Jimmy Page diagram for how to do phase and split together. Splits on JP are on separate switches, but they only use one side of the switch, so you can put both splits onto one switch.
