P-Rail Diagram

Hi all. First post here.

I have a customer that wanted to use all 4 voicing's of his P-Rail Pickup(s). Rail, P-90, Series, Parallel. Pretty much what the Triple Shot Pickups rings do but using Push Pull Pots.
This is the Diagram I came up with after taking with Dennis in the customer service dept.
The Customer wanted the volume pots on top and the tone pots on bottom so that's how it shows in the diagram.
Everything worked perfectly and sounded good in all positions using the amp in my shop and my personal amp, but now he is home using his rig and has informed me there is unbearable buzz. Everything was properly grounded and I cannot figure out what it could be. I don't doubt him as he is a professional musician.
Can anyone go over my diagram to double check if I missed something. Thanks

Doesn't look like the greens ever connect to ground or the whites ever connect to red - but that should result in no sound, so I'm not sure what the issue could be without seeing the actual wiring. He get unbearable buzz all the time? Or just with certain switch positions?
 
Your diagram never puts the pups in series. Series and parallel are the only two humbucking modes.
 
I talked with him this morning and it sounds like the ground wire came loose from the bridge.

ArtieToo I have 4 different voicings for each pickup with it wired like this. Any suggestions on what I can alter to get the desired 4 settings? Thanks for your responses guys
