Can I assume that as a pickup has more gain it also has more distortion/fuzz? Are there any exceptions to this rule? Can you get gain without fuzz?

Pickups don't cause fuzz as much as having too much gain on the preamp or the wrong speakers.

speakers matter 10X more than people give them credit for.

Gain = hotness. Hotter pups transfermore signal to the amp. Think of them as more sensitive, quicker responding, etc. All distortion/fuzz is caused by the amp not being able to handle that much signal. It could happen in the preamp, or the poweramp. Hot pups push a preamp tube over the edge, and a really pushed output tube causes output distortion. Preamp and output distrtion has different qualities. Keep the gain low and the output not too high and you should ge a clean sound. If not, turn the guitar volume down a bit. If none of that works, you need a different amp.

if you run a high output pickup through a high headroom/vintage style amp, it will not distort as much as using a low headroom/hi gain amp.

I agree
top end can give you fuzz
amps and effects give you fuzz
a harsh pup can SEEM to give you fuzz
but Fuzz generally is not your pups fault
