Splitting pickups w/ 5 way switch

My new Duncans are in!! I'm now looking to wire them customly..and stuff.

What I'm looking for is the following:

position 1: neck, series
position 2: neck, split
position 3: N/A
position 4: bridge, split
position 5: bridge, series

This will be on what appears to be a standard 5-way switch (eight poles in-line). How should this be executed?

WOOHOO!!! That sounds like something I'm looking for. I don't want output in the middle position.

The wiring has been completed. I think I wired it backwards the first time since everything was reversed on the switch itself. No worries. I reversed the connections and it runs 100% to my personal specs.
