stc-3m3/smb-4a MM noise hiss problem! I'm going crazy!

This pre-amp is making me crazy. I have an smb-4a through a stc-3m3 in a nice custom jazz body. The bass itself plays great and is set up just ducky. The problem is that the pre-amp is making a terrible hiss! The only way to get rid of the hiss is to roll the treble all the way off, even then I can still "just" hear it. The sound I get when I do cut all the treble is awful, very muddy, and not very useful. The bass is wired correctly, everything works, the bass/mids/volume/slap switch/treble all function, but not without a terrible hissing sound. I've tried this bass through a few different amps, and I've tried those amps with different basses, and the only problem is the electronics within this custom jazz. I think that the pre-amp must be defective, there is no way that this level of noise is normal.
Please, if anyone can help me, I would truely appreciate it.

Thanks in advance

something doesnt sound right. are all the ground wires correct? ive used more than a dozen of them and i have never had that problem. it is possible that you got a defective one.

I checked the wiring against a schematic...it all looks right. What should I try next?
Thanks
