The easiest way to do it from scratch is to connect the input of each tone control to the very same lug for each pickup (bridge and neck) to which you want them assigned.
The portion of the switch that normally assigns tone controls is completely redundant and unnecessary in this particular wiring scenario.
...though there is less work to move just one wire:
