Triple Psyclone Wiring - HHH

Can someone point me to a diagram for very simple HHH wiring diagram that will give me the following options:

1 - Bridge
2 - Bridge + Middle
3 - Bridge + Neck / Toggle to Turn on Middle Alone
4 - Middle + Neck
5 - Neck

Besides the toggle for the middle pickup I want to keep it super simple, no splitting, no series/parallel etc. Thanks and hope all is well.

Sorry guys, I just realized this is basic Nashville wiring. I wasn't thinking Tele, but just connected the dots.
