It sounds like a Mosrite Fuzzrite or a clone. Either the Silicon or Germanium version would work. It depends on how strident you want it.

This is the layout I used to build mine. Mine is a Germanium/Silicon hybrid and I'm not 100% satisfied with it. I always think it sounds cool in the mix but it doesn't gel with the way I play.
