I'll give you the ol' switch by numbers routine, where I refer to lugs by pole and contact. For example, P1.2 refers to pole 1, contact 2. I'll use the same numbers you have above. Both greens (and bares) are hard wired to ground.
P1.C = neck white
P1.1 = open
P1.2 = open
P1.3 = neck red, jumpers to P1.5 and P2.2
P1.4 = ground
P1.5 = jumper from P1.4
P2.C = output
P2.1 = open
P2.2 = jumper from P1.3
P2.3 = jumper to P2.4
P2.4 = jumper to P2.5 and jumper from P2.3
P2.5 = neck black and jumper from P2.4
P3.1 = open
P3.2 = bridge red, bridge white, and jumper to P3.4
P3.3 = open
P3.4 = jumper from P3.2
P3.5 = open
P3.C = ground
P4.1 = bridge black, jumper to P4.2
P4.2 = jumper from P4.1 and jumper to P4.3
P4.3 = jumper from P4.2 and jumper to P4.4
P4.4 = jumper from P4.3
P4.5 = open
P4.C = output
