2 switches to add pickup & change phase

Hi all, I have a weird wiring scheme to figure out. I figured I would ask here because there's so many knowledgeable folks on this forum!

I have a guitar with 2 pickups (and a 3-way switch). I want to add a middle pickup that I can turn on or off, regarless of which setting I have on the 3-way switch. Then I want to add a second switch to change said pickup's phase. It has to be done with 2 seperate slider switches cuz that's the only way it will fit! Thanks if yo can help or point me to a schematic!

Does the new middle pickup need to be on a volume control or just plain on/off? You can just wire it to the output jack in parallel with your other pickups, put an on/off switch on the hot lead followed by a DPDT slider switch for the phase. There's several diagrams of phase switches under the SUPPORT menu on the main Seymour Duncan site. Hope that helps.
