I see one possible problem: where you have your neck green and ground, that terminal needs to be grounded to the back of a pot. That bare wire could also go direct to ground, but it doesn't hurt to have it where it is as long as you ground that terminal. Everything else looks fine. (Well, except for funky solder connections.) :D

One last thing, you probably don't want to mess with this, but if you sent that whole pickgaurd to me, I'd clean up that soldering and wire the whole thing up for you. Free. Just a thought.

BTW your kill switch wiring worked perfect.

update - soldered the terminal with neck green/bare to ground. can still hear all 4 coils when select neck pickup. other position seem good although getting a little "bleed" from dead coils in middle position which I gather is normal?

You should not get any "bleed" in any position. When a coil is deselected, it's shorted out.
The only thing I can think of at this point is that perhaps the contacts on your 5-way are warped or bent due to overheating. It's a long shot, but possible. I have one of my guitars wired this way and it works perfectly.

Just double check everything. If all else fails, you can always fall back on my offer. ;)

Well, then the only position working correctly is the Bridge PUP. When bridge selected, Neck PUP is completely dead.
I'm gonna try re-wiring it standard and see if it works right. I appreciate the offer to wire it for me but it's a project I want to complete this weekend.

I was really excited about the thought of splitting the coils in the middle position since to have both humbuckers on "full" at one time is unusable to me. Lets see if I can get it working and then re-visit. Thanks for all your help!
