How to combine Lil'59 with stock Fender single coils

I need a wiring diagram for

1 Lil '59
2 single coil ( stock fender noiseless )
1 tone
1 volumne
5 way switch

I want to be able to split the Lil 59 in position 4 to use one coil from the humbucker, and the middle single fender stock coil.

Last question - is there anything special I need to do when I use the Seymour Duncan Lil 59 with stock fender coils, as far as phasing? Do I need to flip a magnet?

wire the Lil'59 with it's hot (black) connected to ground and the ground (green) as hot. As far as splitting, I use push/pull pots, I don't know how to do it from the switch.
