P-Rails packaging (instructions?) card ??

Hi folks - I've had P-Rails in 2 guitars for a while and vaguely remember the packaging had some sort of diagram card inside (not a wiring schematic).
Does anyone have a copy of this?
I couldn't find it online or on Seymour's website,
