I'm just curious because I'm inexperienced with tube pedals...

Where would you buy replacements when the tubes for that Mayhem pedal eventually expire and what brand / kind would you need to get?

Also, does the pedal need to warm up like a tube amp does?

I haven't checked but it's probably a 12AX7 valve, and shouldn't need a great deal of warm up time as it's not being used as a power amp.

It's not. It's some kind of 6### valve which are military specs and not made anymore nor available readily. I'm guessing Duncan had to buy millions of the things to service pedals. However any decent preamp valve should last years (if not tens of years) so a late, top of the range, mil spec valve would probably outlast us. Well at least the older dudes on here

Yeh its exactly the same tubes as the Twin Tube and they are meant to last for nearly ever.

They are made to last forever, but it is not exactly the same tube. The TT Classics are Triodes and these are Pentodes. It is the same size though.
