I'm guessing the underside of the guard was shaved down a bit to let the switches stick up enough?
SD definitely could offer a pickguard option, even if it was a CS. I'm thinking Strat, Tele, Ibanez RG, and Les Paul/335 et al, with your choice (for Strat-types) of bass or treble-side mounting, or split, since it's custom. A punch template shouldn't be hard to make. Since it'd be Custom Shop, they wouldn't need a new stamping die with those punches/matrices in it, just a template overlay or "sleeve" that can do each one by hand or like a levered hole-punch. Slide the guard in the slot, pull the handle, and you've got little rectangles around the pickup holes. Mount the boards and you're done.
Of course, any extra necessary woodwork would be on the owner, but then you get into the possibility of having the pickguard formed with the TS already in the layers, with the leads passing through the core to the control section. You'd be limited to 3-ply 'guards, though. Single and 2-ply wouldn't cut it.
