What kind of output jack is this?

Hope in in the right forum.
This cracked and I don't think I can epoxy it back together.
It's from a Yamaha non-import model.

Don't worry. Just put a Switchcraft in it and be happy for the rest of your life!

Pretty much. One nice thing about those open-style jacks is that you can re-tension them if you need to.

That is known as a horrible, wretched POS jack.
Go with the Switchcraft as suggested....
