How could that happen? Did you figure out any specific reason?

Depends on what the 50 watt (speaker) rating represents...peak, rms, program etc. The same goes for the 20 watt amp. When pushed really hard many tube amps can deliver more than their rated amount. You also have to take into account the kind of musical signal is being reproduced. Distorted and thus highly compressed electric guitar can ask a speaker to handle a signal that has relatively little space between notes giving the speaker very little in terms of rest..so the voice coil can over-heat. The most extreme example would be a low note feeding back giving the speaker not a moments rest to cool down.

Have to imagine there was a problem with that speaker more than being overpowered by the amp.

Agreed. Admittedly I don’t know all that much regarding the particulars of how a power amp interacts with a speaker, I just can’t imagine a situation where that may happen...at least in my experience.
I’ve never been in a band situation where I’d need to dime an amp of any wattage.
I have a 20W Blackstar and that amp is at gig volumes at #3 (9 o’clock) on the volume knob.

i cant tell you why it happened but i do assume that it was an anomaly. i had the speaker in the amp for a long time with no issues but i was usually not running the amp full up like that. i got a replacement under warranty so that part worked out fine but when considering what speaker to buy im very conscious of how hot im going to be running the amp.
