Blend for neck and bridge while using 5way switch?

Is it possible to add a blend pot to control the balance between 2 pickups selected using a superswitch?
HSH Desired setup:

1. Neck and bridge humbuckers (with a blend pot control)
2. Neck and mid
3. Mid
4. Mid and bridge humbucker
5. Bridge humbucker

have tried a few things, but everything I've done messes up the position 5, or cuts out sound after the center dent.

if i cant get this to work, I will change the positions to:
1. Neck full
2. neck full and mid
3. neck split and mid
4. mid
5. bridge
separate volume control for bridge, run directly to master, which i think will allow me to mix it into any of the other combinations.
