How difficult or expensive is it to have custom inlays put on a guitar fretboard?

your are probably talking a couple hundred bucks. for example if you wanted black block inlays put on a maple strat board...

pull the frets, sand the finish off, cut the inlays, rout the fingerboard, install the inlays, fret the neck, finish the neck, dress the frets
