Wiring gurus: I need help! (many pics)

Hi forum,

I recently wired a tele (2 HB, seymour duncan bridge, gfs neck) with a 3 way switch, 1 meg ohm volume pot, 500k (which I modified to no load) push/pull switch tone pot. The push pull is wired such that down is parallel for the bridge pickup and up is series; specifically, I modified the wiring recommended by guitarelectronics.com (pictured on the left below) by switching the bottom and top sets of lugs so that down would be parallel instead of series (modification pictured on the right below). Colors represent duncan coloring: black = N. start, white = N. finish, green = S. start, red = S finish. The gold wire is a jumper.

I visually inspected for touching wires etc and checked continuity of ground connections with a multimeter and everything looked fine. However, when plugged in: 1) neck pickup sounds fine, 2) with switch down, bridge pickup sounds ok but tone control is not funcitonal, 3) with switch up, bridge pickup cuts in and out, crackles, and just sounds aweful. tone control works, but at most time appears to be acting as a volume, if that makes any sense. Below are two side views as well as a head-on shot of the push/pull switch.

The only thing different between my picture and the diagram posted earlier is that instead of running the pickup's ground to the right/top-most lug (on the head-on shot) I have a jumper cable running from that lug to the ground on top of the switch and the pickup's ground running to the common ground on the back of the volume pot. But as mentioned all these grounds are continuous so I can't imagine why that would cause problems. Finally, last night, I tried the wiggle wires see what happens approach and: the cable running from the p/u selector to the "hot" lug of the push/pull switch would make ground hum type noises when I touched it. So, I swapped out that particular cable. It didn't help. I also noticed, particularly when the switch is up, that when I touch certain components (e.g., lugs on the switch) there is an onset/offset of hum.
