Wiring help: 3 single coils in a tele, 5-way

This is what I'm after:

1-bridge
2-bridge/mid
3-bridge/neck
4-mid only
5-neck only

I'm also fishing for a strat middle pickup to go well with the JD and Blues special neck. The APS-2 just didn't give me the bite.

Any suggestions?

You'll need a 5 way superswitch... Should be pretty easy to figure out from there...
